check here) and your child has played hockey in another association, please proceed with a transfer based on your scenario:
Scenario 1: Player Moves within the SMHA Boundaries (not sure if you live within our boundaries?
check here)

1. Signed
HEO Attestation of InterBranch/AssociationResidency Form
2. Copy of Purchase and Sale and/or Lease Agreement (1 year minimum lease)
3. Plus Two of the following proof of occupancy:
1. School registration
2. Utility bill, Cable bill (no cell phone) that matches new address
3. Driver’s license (of parent) that matches new address
4. Vehicle insurance and registration that matches new address

Scenario 2: Players who live OUTSIDE SMHA but wish to play in the SMHA:

Note: The Rams Competitive teams do not normally accept players imported from other minor hockey associations. Importing players will only be considered in rare cases where there are an insufficient number of players trying out from SMHA.
