Who requires a police check?

Any member of a team that is on a official team roster: Coach, Assistant coach(s) Trainer(s) and On Ice Helper(s), .

Anyone with a PRC older than 2 years should attend this session.

Requirements

Please bring the completed forms along with 2 pieces of original identification (see list below of acceptable id) between the hours of 6:00 pm and 8:00pm to have your PRC completed.

The Ottawa Police Service has also requested that you bring a photocopy of your ID ‐ it will speed up the process for all involved.

You do not need to pre‐register. However, If you plan to

• Possession and Acquisition Licence (PAL)
• Canadian National Institute for Blind (CNIB)

*Recent Utility Bill required to verify current Ottawa address if no Driver’s Licence is available.
