SMHA requires that all volunteers within our association have a valid police record check. A volunteer is someone who is a head coach, assistant coach, manager, trainer, on-ice helper or member of the executive.

If you have a completed police record check from any volunteering that you have already done, i.e. soccer AND it is dated within the previous 2 years, SMHA will accept it.
If you do not have a completed PRC, then please go to the Ottawa Police Services website and download the form at http://www.ottawapolice.ca/Libraries/Corporate_Speeches_FR/Police_Record_Check_2011_e_web.sflb.ashx

Any volunteer of SMHA can have the fee for this service waived. For the paperwork for this please contact Char McEnery at support.services_2@stittsvilleminorhockey.com
To drop off any completed police record checks, please do so at the Johnny Leroux Stittsville Community Arena where the SMHA mailbox is located in the main lobby. Please place them in the "Risk and Safety" opening.
